Please just mark use of non-OSS software when you make such decisions.
Nearly all nocode simple form-building tools these days have some compromise (usually open-core businesses). Still, even in a space where no OSS option exists at all, the norm of marking proprietary software is part of consciousness-raising, helping people understand the state of OSS.

We used Airtable, here, which is built on inevitably some open source products but is not itself open source. Some of their open source code is available here: Airtable · GitHub.
I would recommend LimeSurvey or NextCloudForms or FramaForms as Open Source questionnaire software alernatives.
